As a parent or guardian, you can complete one of these forms on behalf of your child if they are under 12. If they are over 12 they must complete and sign the form themselves.
Opting Out
If you would prefer not to receive SMS appointment/recall reminders, please inform one of the reception staff of this preference and we will ensure this is recorded.
SystmOnline
If you prefer to use our SystmOnline online service. Simply log-in to your SystmOnline account, then navigate to the ‘Online Account Management’ –> ‘Change Contact Details’ section. Further help can be found in SystmOnline help by clicking here.
